By submitting the online application form, you are digitally signing both the application and the indemnification forms, provided you agreed to this stipulation on the online form.

If you’d rather send your application in by mail,  you can still click on the pdf icons at the bottom of this page and print them on  your printer.  In either case, your passport photo, credentials and your $250.00 mission share deposit check (or full mission share payment) are all necessary to complete your application.

For your safety, medical insurance which will cover medical emergencies during foreign travel is REQUIRED. You may use your existing personal policy or you may purchase one from www.sevencorners.com.  Proof that you have this insurance must be submitted to TFTBM no less than 2 weeks before departure.
